A Springfield nurse is wanted on 14 warrants for charges of endangering patients and patient abuse for reportedly failing to dispense medications to developmentally disabled adults.

Clark County Developmental Disabilities launched an investigation into Carla Howard-Daniels, 37, in July after receiving reports from the director of the Creative Learning Workshop that she allegedly wasn’t providing medications to patients there. The center is a private day facility for adults with developmental disabilities.

One of the county’s investigators allegedly uncovered Howard-Daniels had failed to give required medications to at least seven patients on multiple occasions in July, said Jennifer Rousculp, superintendent for Clark County DD.

The investigation was turned over to Springfield police. Warrants were issued for Howard-Daniels on Friday on seven counts each of patient endangering and patient abuse: neglect. Authorities have not released what the medications were, nor what happened to the drugs that weren’t dispensed.

The investigation is ongoing, and Rousculp said it’s unknown if any patients suffered serious harm.

“If it was a psychotropic or a medication for seizure activity, for diabetes, those type of medical concerns, certainly it could put someone at risk,” she said. “It can be very dangerous.”

Staff at the Creative Learning Workshop declined to comment and wouldn’t confirm if Howard-Daniels is still an employee. A resident at the address listed for Howard-Daniels said she no longer lives there.

She is a licensed practical nurse in the state of Ohio. Her license was listed as expiring Friday, but was noted to be in the active renewal process, according to the Ohio Board of Nursing.

Rousculp said Howard-Daniels’ cooperation in the investigation would be required to keep her license valid. She said the county agency will work with the Creative Learning Workshop to put safeguards in place to prevent similar issues in the future.

“That’s the whole goal of this, is to protect people and protect it from happening again,” she said.
